When you think of the TV show "ER," you might picture scenes of crowded, chaotic hospital emergency rooms.  According to the Journal of the American Medical Association, that's more true than ever.

Researchers say over the past 20 years, the number of E.R's in cities has fallen some 27%.  At the same time, there's been a 35% increase in E.R. visits.  The result, in many cases, is serious overcrowding and, sometimes, sub-par medical care.
